Dr. U uses serial tension stitching which allows the wound to close gradually. This allows the skin to grow, develop and expand to resume the tension and elasticity that it had before, allowing the patient to move their head naturally.

This patient underwent two surgical procedures before the back of his head was finally cleared of Acne Keloidalis Nuchae. During his first surgery, the affected area was excised. However, the physician allowed the surgical wound to heal through the formation of scar tissue as opposed to applying techniques that would allow the excision to close. This is referred to as healing by secondary intention.

However, six months after the procedure, the patient noticed new AKN bumps forming on the scar. He then sought the help of Dr. Umar for a second surgery to clear the reemergence of the new lesions.

The area affected by the recurrent AKN symptoms had to be excised. What Causes Acne Keloidalis Nuchae Growth?

Experts are not clear about the exact causes of Acne Keloidalis Nuchae at this time. However, it seems likely that there are many different factors. A few examples include:

(1) irritation from shirt collars and other types of clothing

(2) ingrown hairs which trigger immune attacks against the follicle

(3) close shaving of the hair with razors.

These variables may affect different individuals to varying degrees.

Although Acne Keloidalis Nuchae is commonly seen in males of African descent, it can also develop in other ethnic groups and individuals who naturally have straight hair.  The patient featured here is an example.

The Progression of Acne Keloidalis Nuchae

In the initial stages, Acne Keloidalis Nuchae looks like acne on the back of the head.  Over time, horizontal plaques and tumor-like masses develop, fusing together into a single bump.

Aside from the issue of cosmetic disfigurement, AKN also causes itchiness, bleeding,  discharge and pain.  While injections can reduce some of these symptoms, they do not clear the lesions.
